Oak Street Health will not be a participating provider at this time in Devoted Health’s Illinois network.
Please ensure you take this into account when assigning PCPs on a Devoted enrollment application. Agents should always refer to the Find a Provider tool to identify providers that are in network. The Devoted Health provider network in Chicago is robust. Please do not hesitate to reach out to Agent Support if you need assistance in identifying participating providers.
